For established Tadler-Hoopes Center for Healing patients. Required either every 6 months or every 12 months according to Utah State law. This is a virtual appointment For your virtual appointment please call 435-572-9469 **In person renewals can be completed upon request for an additional $25 charge**

Cancellation Policy

Thank you for choosing your health. If you cannot keep your appointment, please contact our office 24 hours prior to your appointment to avoid a $25 non refundable fee. No call/No show/missed appointments are subject to a $50 non refundable fee.
