Utah Medical Cannabis Program
QMP is a Qualified Medical Provider who is licensed to prescribe a controlled substance may receive approval from the UDOH to recommend medical cannabis treatment for their patients. These providers must register as a “qualified medical provider” through the Electronic Verification System (EVS)
The Electronic Verification System (EVS) is the Center for Medical Cannabis database used to manage and issue medical cannabis card applications and qualified medical providers (QMP). All patients and QMPs must be registered in the EVS to participate in the program.)

2nd Step: New Patient Card
Other clinics leave you to do all of the paperwork alone. At the beginning of your appointment we walk you through the whole process of setting up your Electronic Verification System (EVS) account within The State of Utah gaining IMMEDIATE ACCESS to your medical cannabis card to be printed and usable the same day as you are scheduled with The Doctor!
3rd Step: Tadler Hoopes Established Medical Cannabis Patients
Its renewal time for your medical Cannabis card. It is required by Utah State for a renewal to be inputted into the EVS system that a Q.M.P. must meet with the patient in order to renew their certification. Please Schedule an appointment for your renewal, we are unable to complete your renewal with out this step.
